Can a provider still bill you or keep your copay/deductible if a claim is denied for timely filing or a provider incorrectly bills the claim for some reason?

Yes they can since they did provide you a service however some insurance companies will not allow a provider/Dr to bill the patient.
The only way to know for sure is to call your insurance company and ask them if you are responsible for the copay. If you are not, ask them to send it to you in writing so you will have something to fight the provider with.
