Transferring Medicaid between states

Question
I have a child who has been placed by the court system in residential facility in SC fro a crime he has committed but the facility is not designed to treat his particular needs. Since his placement I have relocated to NC and have been fighting with the court system for him to receive the proper treatment he desperately needs. I have finally located a placement here in NC that is more than willing to accept him and takes Medicaid but the SC court system keeps telling me that his Medicaid can not be transferred. Since he's covered by Medicaid and would be moved from one facility to another is it possible for his Medicaid to be transferred from SC to NC upon his transfer?

In essence he would be moving from one State to another.  Since each State has it's own Medicaid program, and much of the money is State money, they would not transfer from one State to another.  When he leaves SC his medicaid there would end,  When he reached NC he would have to apply for Medicaid there.
